What is a Care Plan Meeting?

A Care Plan meeting can be requested for any child in care, temporarily or permanently. Facilitators will set up and schedule a meeting with all participants, guide the discussion to fill out individual pieces of the care plan, and put together a summary of the main discussion points.

MCFD and Delegated Aboriginal Agencies can make a referral for a Care Plan meeting.

What is Involved?

The Collaborative Facilitator will:

  • Connect with the Referrer about the individual child and what pieces of the Care Plan need to be filled out

  • Send out an email to all participants (i.e. Band Designates, Social Workers, Caregivers, etc.) to schedule a date for the meeting

  • Guide the conversation to fill in each piece of the Care Plan, keeping everyone on track and within the specified time allotted for the meeting

  • Complete a summary of the main points discussed in the meeting and share with participants

  • Debrief with the Referrer and Social Worker about how the meeting went and if anything can be improved upon for future meetings

What are the outcomes?

  • A completed Care Plan for each child in care

  • More frequent meetings (every 4-6 months) which results in up-to-date information and identification of any gaps or concerns in the child’s ongoing care and growth

  • A focus on making sure cultural needs are being met by including Band Designates and Nation-specific information
