What is an Integrated Case Management (ICM)?

An ICM is a condensed 1-hour meeting that is often used to share information among professionals to identify outstanding safety concerns, strengths/barriers, resource-finding, and provide immediate and focused planning for complex situations. ICM’s are adaptive; they can include planning with youth, families and individuals.

Who Requests an Integrated Case Management (ICM)?

MCFD, Delegated Aboriginal Agencies, community service providers, community members and individuals/families can make a referral for SIWS for a Family Case Planning Conference (FCPC).

What is Involved?

The Collaborative Facilitator will:

  • Work with the MCFD or Delegated Agency Social Worker to clarify safety concerns and confirm the goal of the ICM

  • Identify and connect with necessary community service providers and family members to participate in the ICM

  • Facilitate a collaborative meeting that ensures all participants are actively involved and heard

  • Create goals focused on building safety and addressing outstanding concerns with specific next steps outlined

  • Schedule follow up meetings to ensure goals are being worked on and achieved, tracking successes, and identifying barriers

  • Document each meeting and share the notes with all participants

What are the outcomes?

An ICM results in:

  • A strong team of supports working collaboratively to identify outstanding safety concerns, strengths/barriers, assessing risks and sharing information. ICM’s outline which professional is best suited to address each issue/goal.

  • Avoidance of any triangulation or miscommunication between large groups of supports

  • Condensed time-frame to a 1-hour meeting, more adaptable to professionals

  • Open communication about concerns during discussion and planning

  • With positive growth from impacted individuals, meetings can often transition into Family Case Planning Conferences and include individuals in planning and next steps

  • A written, approved action plan that all parties follow
